Your health record includes information like any conditions or allergies you have and any medicine you’re taking.
Most patients will automatically have access to more information added to their health record. This includes letters, test results and appointment notes.
You can access your health record and nominate someone you trust to access it too.
You can use your NHS account (through the NHS website or NHS App) or SystmOnline to access your health record.
You’ll need to ask the surgery for online access to your full health record, or you’ll only see your medicines and allergies.
PATCHS
PATCHS offers a secure, simple and flexible way for patients and carers to contact their GP practice – it will not replace face to face appointments, it is in addition to routine contact with Practices. This should not be used for emergencies when you should contact 999 or visit A&E.
By registering you will have easy access to a range of services from your computer or smartphone, including:
- reporting symptoms to your GP
- accessing health advice
To request repeat prescriptions, you should continue to follow the current process we have in place.
All you will need to do is provide an email address and set up a password. Please make sure when you create your account that your first name and surname exactly match the details you used when you registered with the practice.
If you have any problems registering your account, you can call the surgery who can help with this. The initial set-up might take a few minutes, but once you have an account, we hope you will find the new platform a lot more streamlined, and easier to use.
To register visit register with PATCHS, Ashville Surgery.
For more information on PATCHS you can visit their website.
Access to other medical records
For any other medical records, you need to ask for them at the NHS service you went to. This includes records like your hospital record.
