How to register as a new patient online
We are using a new online service called Register with a GP Surgery. That makes it easy to register with Ashville Surgery. Scan the QR code or follow this link.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
(Paper forms are still available if you need one)
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Just outside the boundary?
If you live outside this boundary, you will normally need to register with a practice closer to your address — you can find alternatives using the NHS “Find a GP” service.
You may still be able to register with us if you’re within a reasonable distance. Complete our Out of Catchment Area Registration Form, along with the standard Registration Form, and return both to the surgery via email (ashville.surgery@nhs.net) or post or in person.

Please note: if you register out of area, we won’t be able to offer home visits — you’d need to arrange this through a local service if needed.
Registering children
For children under 6 (excluding newborns), please bring their immunisation history (Red Book or equivalent) — including for any vaccines given privately or overseas, so we can add these to their record.
Children can only be registered if at least one parent or guardian is also registered with us, or registers at the same time.
Change of address
If you are moving, please let us know as soon as possible. If you’re relocating outside the surgery’s catchment area, you may need to register with a surgery closer to your new home.
To update your address, telephone number, or email, we highly recommend using PATCHS for a quick and easy process.

Temporary residents
Unwell while away from home, or not registered with a GP but need to see one? You can get emergency treatment from your local practice for up to 14 days before needing to register as a temporary or permanent patient.
You can register as a temporary resident with a GP surgery for up to 3 months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with the surgery.
To register you’ll need to fill out a temporary services form (GMS3), available from the surgery. Please return the completed form to the surgery once completed.
