Enhancing patient access into general practice – the NWL model

Across North West London, general practices and Primary Care Networks were asked to develop an access model that enhances patients’ access to GP surgeries. The aims of the specification that North West London developed are to make effective use of resources, increase use of digital tools and increase proactive care for those that need it.  

As part of the specification we submitted a delivery plan, which includes:  

  • implementing a care navigation programme to ensure that all staff, including reception and administrative, are able to support patients in seeing the most appropriate person for their needs.
  • expanding our successful prostate cancer detection pilot.
  • increasing engagement and awareness of PCN services and Pharmacy First through the creation of media content including videos, leaflets and PPG presentations.
  • developing education workshops for local residents and patients on the NHS App and PATCHS.
  • continuing to harness the roles of the PCN Clinical Leads who lead service redesign and ensure the PCN is fully collaborating with the wider health system, for example local hospital and community trusts.  Our clinical lead areas include Mental Health, Women’s Health, CYP, LD, Prescribing, Diabetes and Frailty.
  • delivering further health / community events focusing on specific health areas or cohorts of our local population to tackle health inequalities and support our communities’ needs.
